Do you have Capital Health Plan?
If so you can be reimbursed up to $150 for your yoga classes.

- the CHP reimbursement form available on their website
- your canceled checks or a receipt from Journeys– which you can print out by logging into our MindBody system, upper right hand corner.
Send these to the address on the website. You have until March 31st, 2013 to get reimbursed for 2012. You can only submit one time each year, so for 2012 wait until you have spent $150.
